2. Step -- Follow the link to the questionnaire

3. Step -- Fill the questionnaire

• press the insert button. The information you provided is displayed so that you can verify it

• press the insert button again. Now the form is inserted in the database

• NOTE: once finalised and inserted in the database you cannot delete or modify the information. If for any reason you need to do that please send an e-mail to kolb(at)eurescom.eu and explain your request.

As an alternative to the steps above you can download the word file, fill the questionnaire and mail it to kolb(at)eurescom.eu
